360 Youth Services is seeking an Intake and Aftercare Manager to support youth experiencing homelessness to enroll in housing services. The candidate must be energetic, engaging and interested in fostering a welcoming, trauma-informed space for youth, ages 18-24, who are experiencing homelessness. The Intake and After Care Specialist is responsible for being the first point of contact for youth and community referrals who are seeking shelter. They will explain the programming options to young people, conduct phone screens and bio/psycho/social assessments and complete necessary paperwork to triage and enroll youth in Emergency Shelter or other appropriate housing programs at 360 Youth Services or in the community. They will develop and maintain relationships with appropriate referral sources, maintain contact with housing clients who are waiting for shelter and coordinate entry into program with the Program Managers and participate in exit and after care planning for residents to exit to ultimate goal of helping residents prepare for permanent housing options including transitional, rapid rehousing, or other stable housing opportunities.
