The Post-Acute Care Nurse Manager is responsible for the implementation, monitoring, and supervision of all clinical aspects of the short-term, post-acute care/rehab floor. In addition, this position is also responsible for oversight, direction, supervision and evaluation of all professional and certified nursing staff within the neighborhoods. Ensures compliance with all federal, state and local regulations including corporate compliance and HIPAA Privacy Standards. Salary is $80,000 – $90,000 and a $3,600 sign on bonus. The following lists some responsibilities of the position:

  • Responsible for creating and maintaining a person-centered culture through interacting with residents and staff, participating in shift-to-shift report and through direct observation. Upholds standards of care that demonstrate sound clinical practice, while also emphasizing a person-centered care approach.
  • Performs or oversees nursing assessments on admission, quarterly, and with change in condition as indicated. Assessments include all clinical programs, such as pain, skin, bowel and bladder, fall risk, mood and behavior and mobility. Consults with all appropriate resources in developing resident plan of care and updating daily as needed.
  • Coordinates care with the Rehab Department to ensure therapy needs are being met as ordered and determined. Acts as the guest liaison between Nursing and Therapy as needed.
  • Participates in the weekly Medicare Meeting, and occasionally in short-term Care Plan meetings when there is a difficult case and/or the MDS staff are unable to participate.
  • Responsible for monitoring and teaching nurses about appropriate documentation associated with Medicare/Managed Care guests.
  • Manages guest processes such as medical follow-up appointments, medication management and the nursing discharge process.
  • Maintains a professional relationship with the facility APN, Short-term Physicians, and the Medical Director.
  • Ensures all care and services are delivered to each resident per his/her individualized plan of care. Responsible for ensuring services are documented daily within the medical record.
  • Assists in hiring, orientation and training of staff, observes and evaluates performance to ensure quality standards and service, and completes investigations and disciplinary write-ups.
  • Serves as member of nursing leadership team, including participation in Nursing Manager on Duty rotation. Actively seeks out Administrative support when not sure of how to resolve an issue.
