| Right-click on the "Service Card Application" button and: |

do "Save Target As..." if using Internet Explorer, or |
|
|
| |
do "Save Link As..." if using Firefox. |
|
|
| Use the resulting Save dialog box to save the application to your computer's desktop. |

|
Fill out all of the blank fields on the form.
