Complete and Return our Intake Information Form
To use any of our Advanced Services – attending job search workshops, meeting with a Counselor, or applying for a WIA job training grant – you will need to complete an Intake Information Form and get an Intake appointment. (The form is in PDF format, which requires Adobe Reader.) |

do "Save Target As..." if using Internet Explorer, or |

do "Save Link As..." if using Firefox. |
|
|
| Use the resulting Save dialog box to save the application to your computer's desktop. |

Fill out all of the blank fields on the form.
Once completed, return the PDF to us by:
