Getting Started
 
1. Get a Service Card – To use any of our Basic Services you only need to get a Service card. Come in to our center to fill out an application form, or save time and download it now.

 A dialog box will pop up asking you to open or save the form; choose Save.
 Select a location to save the form (we recommend selecting the desktop.)
 Open the form and complete it by typing in the form fields. Be sure to check all applicable boxes.
  Once completed, please fax or email your form to:
   Fax:    (630) 955-2059, Attention: CSR
   Email:  contactus@worknetdupage.org  (Please attach your completed Service Card form to your email.)
   
2. Get an Intake Information form – If you are interested in attending workshops, meeting with a Counselor, or applying for a job training grant (Advanced Services), you will need to complete both the Service Card Form above and an Intake Information Form.

  What Happens Next?
 

Once we have received your forms, you will be contacted to schedule an Intake appointment.

Following your Intake appointment, an Assessment appointment will be set up with a Counselor to determine your eligibility for scholarship funding. Our scholarships are funded through the Federal Workforce Investment Act (WIA) and can be used for approved types of occupational or vocational training.

   
  What Else Can I Do?
 

While you are waiting for your Assessment appointment, we ask that you start to research your training needs so that a solid employment plan can be quickly developed. Before your Assessment appointment, please research and visit several schools that provide the training you need. Consider the following questions:

  checkmark Is the school I’m considering on the approved list of training providers?  (The approved provider list can be found on our website or by clicking here.)
  checkmark How much does the program cost?
  checkmark Is this training necessary for me to become employed?  (If it is, please be prepared to tell your Counselor why.)
  checkmark Will I be able to find a job related to this program after I finish training? Is this type of job in-demand? (Labor market data must support your employment and training goal. To research in-demand occupations and labor market data, go to www.illinoisworknet.com and search Key Sectors.)

By doing this research before your Assessment appointment, we will be able to assess your training and employment plans more quickly and hopefully prevent any delay in starting training.
